What type of parental consent is needed for cruise ship travel?

My parents are requesting that my pre-teen daughter be allowed to accompany them on a cruise to South America. While I think it would be a great experience for her, and I know my parents would really enjoy spending quality time getting to know their granddaughter, I’m wondering what is required for her to travel outside the country without one of us, her parents. Does anyone have information on what types of forms on parental consent for travel on cruise ships are required? I want to make sure everything is in order before they book to avoid any problems down the road.

There is a form that is required by all cruise lines, a form that parents are required to complete if a minor child is traveling out of the country without one of the parents with them. It is called “Letter of Parental Consent for minor child to Travel.” Information such as the names of all parties involved, where they are traveling to and by what mode of transportation, and dates will be needed on the consent for it be complete. After the parents complete the form, it has to be notarized. I would suggest making several copies of the consent form as well, in the event one is misplaced. You can download this form from the Internet, but personally, I would check with the cruise line first to see if they have a specific form they require.
